What to expect:

-You can schedule your appointment on this site, on the home page. Or you can call 303-485-1922.

-If you are a new patient, you will get a text/email with a link for the patient intake paperwork and consents. It will be uploaded to your profile directly. Please fill this out before your appointment.

-If you are unable to do the paperwork online, please show up to your appointment 15 minutes early to do the paper copy in the office before your appointment.

-A deposit is required for a new patient exam and a DOT physical. Failure to show is a forfeiture of the deposit. Please give 24 hours notice.
